Admitting Diagnosis: Multiple Sclerosis
Discharge Location: Home

A 57-year-old female was admitted to Briarwood Rehabilitation and Healthcare Center from Brigham and Women’s Faulkner Hospital for continued skilled nursing and rehab needs. Upon admission, Mrs. F. was evaluated by and participated in skilled occupational and physical therapy where she presented with decreased strength, balance, and overall functional mobility. She was also below her baseline for her ability to perform self-care tasks. She was noted to require minimum assistance with toileting and lower body dressing tasks, limited by left lower extremity weakness and bilateral lower extremity edema, and she was also noted to require supervision with bed mobility, transfers and ambulating 150 feet with a rolling walker. During her time at Briarwood, Mrs. F. was always willing to participate in therapy and she was very determined to reach her goal of going home. Upon discharge, Mrs. F. was able to perform all self-care tasks independently, improved her activity tolerance, strength and balance leading to her being able to ambulate 300 feet with a single point cane independently and was able to perform bed mobility, transfer and ascend/descend 14 stairs independently. She made great gains toward her therapy goals and was able to return home with home health services. Wonderful job Mrs. F.

CASE STUDY

Admitting Diagnosis: Covid-19; Acute Respiratory Failure with Hypoxia
Discharge Location: Home

A 75-year-old male who was admitted to Briarwood Rehabilitation and Healthcare Center from Rhode Island Hospital for continued skilled nursing and rehab needs after being diagnosed with Covid-19 and Respiratory Failure. Upon admission, he was evaluated by and participated in skilled occupational and physical therapy where he presented with a decline in his ability to perform his self-care tasks and unsteadiness on his feet. He was noted to require between supervision and minimum assistance for most self-care tasks, setup assistance for bed mobility and supervision to transfer and ambulate 40 feet. He was also noted to have decreased strength and activity tolerance and impaired balance. During his time at Briarwood, he was very motivated to reach his goal of going home and he was always willing to participate in his therapy. Upon discharge, Mr. D. was able to make marked improvements with activity tolerance, strength and balance and was noted to be able to complete all self-care tasks independently and was able to perform bed mobility, transfer, ambulate 200 feet and ascend/descend more than 15 stairs independently. He made great gains toward his therapy goals and was able to return home with strong family support and home health services. Great job Mr. D.
