Connectivity and Engagement are at the heart of our commitment to Caring Safely.
Virtual Engagement via Skype, Zoom and other platforms provide our residents and family members reassurance and peace of mind. Real Voice telephonic updates provide frequent communication to our family members on any changes in our Care Safely Procedures and Protocols. Monthly Medical Director led Zoom sessions provide loved ones the opportunity to speak directly with our Facility Medical Leadership.

Post-Discharge Follow Up Ensuring Safety & Engagement At Home.
To safely navigate this transition period, we offer:
- Transitional Planning Management focused on reducing unnecessary exposure to potential infections via multiple home care providers
- Therapy-Driven Virtual Home Visits to support increased independence and continued functional maintenance post-transition
- Restore To Home, a Virtual /App-Based Game Therapeutic Exercise Program that allows the family to join in the fun and participate whether in person or via virtual. To ensure success, a therapist checks in weekly and offers continued guidance & support through 31 days post-discharge!
